• Registration fees include participation in scientific sessions, forum materials, coffee/tea, lunch symposia and Satellite Symposium cum Gala Dinner on 19 May
• The registration fees listed in US Dollar (US$) are for reference only.  Registration fees will be charged in local currency (Hong Kong Dollars).

^ Verification letter issued by hospital is required for delegates who register as Trainee.

Payment 
Payment must be made in Hong Kong Dollars (HK$) by the following methods:

1. Cheque: payable to “International Conference Consultants Limited”
2. Credit Card: Visa or Master Card

Registration confirmation will be emailed to the registered delegates after receipt of full payment.

Cancellation Policy
Cancellations must be made in writing to the Forum Secretariat and the refund will be made after the Forum.  If cancellation is received on or before 28 February 2017, 50% of the registration fee will be refunded.  Thereafter, there is no refund of registration fee.

Rules and Regulations:

1. Registration form without payment will NOT be processed.  Please do NOT send cash.
2. Secretariat will send a letter of confirmation by email upon receipt of your registration form and full payment.  Any changes or alterations must be made in writing to the Secretariat.
3. Refund for cancellation of registration will be made and subject to the following deadline and administration charge.
   All cancellations must be made in writing to the Secretariat and the refund will be made after the Forum.

On/before 28 February 2017	50% of the registration fee
After 28 February 2017	No Refund

4. The Programme is subject to change without prior notice.  In the event of cancellation of the Forum, the only liability of the Organizers is to refund all the fees paid.
5. All enquiries, changes and cancellation should be made in writing to the Secretariat.
